Gym Membership Reimbursement Program

Individual Membership
8 times a month = 50% up to $40.00
12 times a month = 100% up to $40.00

Family Membership
12 times a month = 50% up to $80.00
18 times a month = 100% up to $80.00

Mo-Kan will only reimburse for the actual gym membership fee. Any additional fees your gym may charge are not payable under this benefit. Tanning beds or saunas sessions are not payable under this benefit.

Please submit a paid receipt for each month and your monthly attendance record*. Be sure to write your Mo-Kan ID number (on your BCBS card) on the claim. (*If you have a family membership, remember to include an attendance record for each family member that is working out. Family members must also be a dependent on your Mo-Kan plan. Do not include attendance records for dependent children that are too young to work out or are at the gym for childcare.)
